CIY Mix 2026

*Please fill out the first section with YOUR information and all the others with your STUDENT'S information* Students in 6th-8th grades during the 2026/27 school year are invited to CIY Mix with Sherwood Middle! We will be going to Taylor University on June 28-July 2, 2026.   You will receive a payment email as soon as this form is submitted.

Please notice that this is not the current grade your student is in, but the grade they will be in for the 2026-2027 school year as this event takes place after the current school year has ended.
